Joining SwiftMD takes just a few minutes.
Once you’ve joined, you’ll never have to fill out a basic health form for SwiftMD again. To enroll, you’ll need the following:
- Contact information, such as address, phone number and email
- Personal information, such as your age, height and weight
- Basic health history, such as you’d provide at the doctor’s office
- That same information for each person in your plan (if applicable)
- Your credit card of choice
Once you complete the enrollment form, you’ll be assigned a secure user name and password so you can access your Personal Health Record and other free member benefits. Please take the time to login and complete your medical history form so it can become part of your record. When you need to talk to a doctor, you’ll be ready to do so. It’s that simple.
