Joining SwiftMD takes just a few minutes.
Registering is easy.
Once you’ve joined, you’ll never have to fill out a basic health form for SwiftMD again. To enroll, you’ll need the following:
- Contact information, such as address, phone number and email
- Personal information, such as your age, height and weight
- Basic health history, such as you’d provide at the doctor’s office
- That same information for each person in your plan (if applicable)
- Your credit card of choice
When you complete the enrollment form, you’ll choose a secure user name and password so you can make appointments online, and access your Personal Health Record and other free member benefits. Please take the time to login and complete your Medical History too so it can become part of your record.
When you need to talk to a SwiftMD doctor:
When you make an appointment by phone or Internet, you will answer a few simple questions about your condition to make sure it isn’t an emergency. If it is, we'll recommend the appropriate provider, whether that’s a doctor or your nearest hospital Emergency Department. If not, we'll schedule an appointment for you.
An emergency-trained physician calls you back ASAP:
Our physicians call members back, usually within 30 minutes. The physician asks you about symptoms, pain and other issues. You also have the option to video-conference with the doctor if you like.
You are provided with a course of treatment:
Our physicians can prescribe medications, order lab tests, make referrals and more. If prescriptions or tests are required, we facilitate everything right down to calling in the prescription to a pharmacy near you.
